So, as Ivashkin asked, why is it dangerous to the user to fire? I had a quick look at the images and when looking fuze it's purely setback armed and inertia fired. I guess you could easily fire a round that is already armed, or even drop an armed round?

the danger is not the fuze.
1. The launcher tube had been manufactured from inferior material. The investigation of several incidents revealed that there was a lot of intrusions in the metal (like enamel from the melted pots used as scrap metal). Causing it to desintegrate under pressure of gasses.
2. the stabilizer tube (no.2 at the picture) is attached to the warhead by a fine thread. Upon impact it tends to break away and return in the direction of the shooter.
3. The fine steel dust used as counterweight was also somewhat unplesant to the shooters friendlies.
