5.56 mm fired case identification

Can anyone help with this one (with apologies for the picture quality)? It is a 5.56x45 case, apparently made by Lake City in 2011. I understand that the three circular depressions are known as SCAMP marks ("Small Caliber Ammunition Modernization Program") which indicate which machine the cases were loaded by. Apart from the NATO symbol, the other mark on the base was I believe made by the weapon which fired it.

Can anyone confirm this, with bonus marks given for identifying the type of weapon? :tinysmile_classes_t
