Problem with VOG-25P

It happened two days ago in Ukraine, during practice shooting in university. Soldier in hospital, grenade disarmed on polygon.
What exactly happened - no news yet. Everything on photos.

I saw the same pic posted by STRAKV, wonder if if the firing pin is stuck, then during reloading the user slams it in causing a premature firing.

I have a VOG-25, is there a place I can get a metal replacement cap and a plastic shipping cap?

It fires from the GP-25, it is loaded from the muzzle.

He shot himself in the belly but it was fortunately also a prac version!
