US MKII Red fiber Washers

Can anyone provide a time line for use of the red fiber washer and/or the black rubber washer used on the US MKII grenades?

Also, in conversations I have had with other collectors over the last couple years it has been mentioned that there is a book coming that supposedly documents and details all U.S. grenades. Does anyone know the phantom author or when this mythical book will be available? Seems to be a long time in coming!
