How much lives can be wrecked before someone higher up the foodchain steps in and takes responsibility ???

It usually takes them till the end of the conflict to do anything worthwhile, and then the "zero defect" mentality sets in stating that no one can get hurt or heads will roll. Heard a lot of the same from our side of the pond dealing with EOD. People in position of authority that have no concept of what EOD does let alone how to spell ordnance. The poor guys kit was probably back in Ireland, "where it was needed" (heavy sarcasm intended).
