Does someone have info about where the load assembly and pack (LAP) of this munition took place, i.e. which GOCO facility in the USA?

Does someone have any idea who made the mine itself or its parts?

And who can tell me in which theaters this munition was used and when it was procured?

looks like Louisiana Ordnance Plant...

but I doubt they made the mine itself, probably just filled it.
