Why did this avatar appear and disappear ? Very strange, no !?
 
The avatar is simply a link to an image on the webserver, Until I replaced the image recently, there was simply a blank image in its place.

People's browser are likely to have the old image 'cached' so it doesn't have to download it everytime a page is accessed.

The only way to ensure you are looking at the latest version of that image would be to tell your browser to refresh a page and all its contents so it replaces anything in its cache with the latest version. Hence doing a CTRL + R sorted it out.
